This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.
Novos Leaf is a HIPAA Business Associate that provides technology infrastructure to covered healthcare providers. This Notice applies to Protected Health Information (PHI) handled through the Novos Leaf platform in connection with medical cannabis telemedicine services.
Treatment: We use and disclose your PHI to facilitate your medical cannabis consultations, including sharing your intake information with your treating provider and transmitting your recommendation to state registries.
Payment: We may use your PHI to process payments for telemedicine consultations.
Healthcare Operations: We may use de-identified or aggregated PHI to improve our AI models, quality assurance, and platform performance.
Required by Law: We will disclose your PHI when required to do so by federal or state law.
Other uses require your written authorization.
Right to Access: You have the right to inspect and obtain a copy of your PHI maintained in our systems. Submit requests to legal@novosleaf.com.
Right to Amend: You may request that we amend your PHI if you believe it is inaccurate or incomplete.
Right to an Accounting of Disclosures: You may request a list of disclosures of your PHI made in the past six years.
Right to Request Restrictions: You may request restrictions on how we use or disclose your PHI. We will consider your request but are not always required to comply.
Right to Confidential Communications: You may request that we contact you about your PHI in a specific way or at a specific location.
Right to a Paper Copy: You have the right to a paper copy of this Notice upon request.
Novos Leaf is required to: maintain the privacy of your PHI; provide you with this Notice of our privacy practices; notify you if a breach of your unsecured PHI occurs; abide by the terms of this Notice; and not use or disclose your PHI other than as described here or with your written authorization.
If you believe your privacy rights have been violated, you may file a complaint with Novos Leaf at legal@novosleaf.com, or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/privacy. We will not retaliate against you for filing a complaint.

This Notice is effective January 1, 2025. We reserve the right to change this Notice and to make the revised Notice effective for PHI we already hold.
